Students who have been found by their physicians to be incompatible with scuba diving will need to upload the completed Padi medical questionnaire stating from the primary care that: Not approved – I find conditions that I consider incompatible with recreational scuba diving or freediving.Make sure the physician's name and contact information is legible.
