The RSTC Medical Statement and Guidelines for Recreational Scuba Diver’s Physical Examination has been emailed to you in the form of a .pdf. Have your physician sign this document clearing or not clearing you to take part in this program. You must Bring the signed form back to Scuba Fusion before the start of your program.
